Intake Coordinator

Job Summary

The Intake Coordinator plays a vital role in ensuring the smooth flow of new patient referrals within our home health agency. This position involves coordinating with various stakeholders, including patients, families, referral sources, and the interdisciplinary team, to facilitate a seamless transition into our care.

Responsibilities
  • Manage all daily referral intake operations, ensuring timely processing and documentation of new referrals.
  • Conduct clinical reviews of referrals to verify completeness and appropriateness for admission based on agency guidelines and formulary.
  • Coordinate with insurance providers to obtain authorizations and verify patient eligibility.
  • Communicate effectively with patients, families, referral sources, and internal teams to provide updates and address inquiries.
  • Develop and maintain efficient systems for the intake process, striving for continuous improvement.
  • Collaborate with clinical managers, supervisors, and other departments to ensure seamless patient care transitions.
Qualifications
  • Graduate of an accredited school of vocational/practical nursing (preferred) or High School Diploma/GED.
  • Active state license as a licensed vocational/practical nurse (preferred).
  • One year of experience as a licensed vocational/practical nurse; home health experience is highly valued.
  • Proficiency in medical terminology and healthcare documentation practices.
  • Excellent communication, interpersonal, and organizational skills.
