Job Title: Case Manager

CAMBA is seeking a skilled Case Manager to join our team at the Bergen-Saratoga Supportive Housing development in Brooklyn. As a Case Manager, you will play a critical role in supporting our clients in achieving their goals and improving their overall well-being.

Job Summary:

The Case Manager will be responsible for providing case management services to clients, including conducting initial intakes and assessments, developing and implementing individualized service plans, and connecting clients with community resources. The ideal candidate will have experience working with vulnerable populations, excellent communication and interpersonal skills, and the ability to work independently and as part of a team.

Responsibilities:
  • Conduct initial intakes and assessments of clients to identify their needs and goals
  • Develop and implement individualized service plans to help clients achieve their goals
  • Connect clients with community resources, such as healthcare services, education and employment opportunities, and housing assistance
  • Provide ongoing support and case management services to clients, including regular check-ins and progress monitoring
  • Collaborate with other service providers and stakeholders to ensure a comprehensive and coordinated approach to client care
  • Maintain accurate and up-to-date records of client interactions and progress
  • Participate in case conferences and other meetings as needed
Requirements:
  • Bachelor's degree in a related field, such as social work or psychology
  • Experience working with vulnerable populations, such as the Seriously and Persistently Mentally Ill (SPMI) population
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work occasional evenings and weekends
