About the Role

Housing Works is seeking a skilled Licensed Master Social Worker/Case Manager to join our Adult Day Health Care (ADHC) Program in Brooklyn. As a key member of our interdisciplinary team, you will provide mental health/substance use/supportive counseling, crisis interventions, and case management services to clients living with chronic illnesses, including HIV, behavioral health conditions, diabetes, hypertension, and others.

Responsibilities
  • Manage a caseload of 30-40 active clients, providing individualized treatment goals and objectives
  • Conduct initial assessments and quarterly/bi-annual reassessments to develop and update treatment plans
  • Facilitate up to 6 groups per week, promoting evidence-based interventions and harm reduction models
  • Collaborate with the interdisciplinary team to coordinate multidisciplinary services and ensure timely submission of clinical documentation
  • Provide crisis intervention services and refer clients for psychiatric evaluations and emergency services as needed
  • Support advocacy efforts to end homelessness and AIDS
Requirements
  • NYS Licensed Master Social Worker required
  • More than 2 years of experience working with individuals or families living with mental illness, substance use, homelessness, or HIV/AIDS
  • Previous experience in evidence-based group interventions preferred but not required
  • Ability to use data to inform clinical decision-making and apply population health and Continuous Quality Improvement principles
