Job Summary

University Hospitals seeks a highly skilled and experienced Sepsis and COPD Coordinator to join our team. As a key member of our healthcare team, you will be responsible for coordinating patient care services for patients with suspected sepsis or confirmed sepsis. Your expertise will be instrumental in promoting care delivery that ensures efficiency, clinical outcomes, and cost-effectiveness.

Key Responsibilities
  • Direct patient care and assessment, ensuring Relationship-Based Care
  • Maintain accurate records and provide staff education and development
  • Develop and implement discharge planning, community resource referrals, and education
  • Provide exceptional customer service and initiate positive relationships with patients, customers, and coworkers
  • Take responsibility for self-development and support a learning environment
  • Display commitment to the mission of University Hospitals and its values
  • Develop and maintain care standards, program development, and ongoing program maintenance
  • Design teaching programs for patients, families, and nursing staff, and provide assistance in program implementation
  • Act as a sepsis resource for nursing and direct care providers, implementing and enhancing patient education activities
  • Provide specialized nursing knowledge and expertise as a clinician, educator, researcher, and consultant
  • Oversee sepsis program development in conjunction with the Medical Director and CNO
  • Develop policies and procedures, facilitate performance improvement teams for sepsis, and ensure compliance with accrediting agencies
  • Perform daily and concurrent reviews of patients treated with sepsis protocol for adherence, performance, and outcomes
  • Collect and analyze data, recommending action plans to the team
  • Represent sepsis care on departmental and hospital task forces
  • Assure delivery of Best Practice Standards for sepsis care and participate in program education, outreach, and research activities
  • Develop effective presentation tools to facilitate, interpret, and analyze sepsis data
  • Facilitate sepsis interdisciplinary meetings, develop agendas, and quality review processes with medical directors and CNO
  • Collaborate with marketing to develop community outreach and educational programs
  • Actively participate in sepsis performance improvement programs, including data analysis, medical record extrapolation, and data entry
  • Identify areas for nursing research and grant proposal writing in collaboration with the CNO
  • Ensure compliance with regulatory bodies and monitor tracking entry analysis data interpretation
  • Provide education for physicians, nurses, hospital staff, and community members
  • Action planning for sepsis and research and grant proposal writing
Requirements
  • Bachelor's Degree in Nursing (BSN) and 3-5 years of progressive clinical and management experience in nursing
  • 3+ years of experience in an ED or inpatient setting and expertise in public speaking, oral and written presentation skills
  • Progressive knowledge in Neurology, data management, and outcomes management
  • Expertise in the coordination of patient services and care for sepsis
  • Strong knowledge of technology and software applications
  • Skills in problem-solving, communication, teaching, and physical assessment
  • Strong interpersonal skills, leadership ability, and experience as a nursing and pre-hospital educator
  • Registered Nurse (RN), Ohio and/or Multi-State Compact License Upon Hire and Basic Life Support (BLS)
