Job Summary

We are seeking a highly skilled Clinical Intake Coordinator to join our team. As a Clinical Intake Coordinator, you will be responsible for facilitating and planning referral, intake, and admission processes for incoming patients. You will work under the general supervision of the Director of Admissions or Facility CEO and will be based at our facility location in Phoenix.

Responsibilities
  • Perform telephone preadmission screenings and clinical assessments
  • Verify insurance benefits and assist with pre-certification as needed
  • Respond to inquiries and referrals regarding facility admissions and communicate provider recommendations to community partners, patients, and their families
  • Educate clients, family members, referral sources, and hospital personnel about hospital benefits and services
  • Schedule and complete pre-admission, intake assessments, and all other intake paperwork
  • Communicate with facility and company medical and psychiatric personnel to ensure appropriate placement recommendations and admissions
  • Provide accurate and ongoing assessments of patient status in referral, admissions, and intake processes
  • Perform ongoing assessments and physical function, emotional, social, spiritual, and financial needs on patients and implement crisis intervention and referral
  • Work with the Business Development team to assess patient needs and initiate, implement, and evaluate individualized patient plans
  • Maintain a professional approach with confidentiality and assure protection and privacy of health information
Requirements
  • Active RN License or Master's Degree in Social Work, Counseling, or a closely related Social Services Field required
  • Minimum of six months in psychiatric and/or behavioral intake department and/or six months in psychiatric and/or behavioral Business Development and/or Marketing required
  • Must demonstrate expertise in analysis, decision making, time management, oral and written communication skills, computer use, customer service skills, and strong ability to function efficiently and effectively in a high stress and fast paced environment
