About the Role

We are seeking an experienced Nurse Care Coordinator and Case Manager to join our team in Texas. As a key member of our interdisciplinary care delivery team, you will play a vital role in coordinating patient care and providing ongoing nursing assessments.

Job Summary

The successful candidate will be responsible for performing home visits and telephonic case management, working closely with patients, families, caregivers, and healthcare providers to ensure comprehensive and coordinated care. You will also be required to document progress toward established goals and maintain accurate records.

Key Responsibilities:
  • Conduct medical home visits on established patients, performing hands-on exams at each visit.
  • Provide ongoing assessments of the impact of life-limiting and/or chronic illnesses on patients' physical, functional, psychosocial, and environmental needs.
  • Assess caregiver burnout and provide community resources when necessary.
  • Implement individualized plans of care and recommend revisions as needed.
  • Perform procedures such as wound care, phlebotomy, and injections (B12, vaccines).
  • Conduct advanced care discussions with patients and families, documenting their wishes in the electronic medical record.
  • Initiate preventive and rehabilitative nursing procedures as appropriate.
  • Ensure continual assessment of patient and family needs throughout the course of care.
Requirements
  • Registered Nurse with a current license to practice in the state of employment.
  • Current BLS certification or must obtain certification within 30 days of employment hire date.
  • 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
  • Excellent nursing skills and ability to interact productively with individuals and multidisciplinary teams.
  • Reliable transportation for daily travel to various locations as assigned.
Salary and Benefits
