About the Job


We are seeking a skilled Care Coordinator Specialist to join our team at the Texas Department of Aging & Disability Services. As a specialist in the Program Support Unit (PSU), you will perform moderately complex tasks related to managed care Long Term Services and Supports (LTSS). This role involves frequent contact with various stakeholders, including case managers, service providers, Managed Care Compliance and Operations, and Medicaid for the Elderly and People with Disabilities (MEPD) staff.



Key Responsibilities



  • Develop, coordinate, and explain Medicaid waiver programs to applicants and members, authorizing or denying waiver services as needed
  • Validate eligibility in appropriate systems, register waiver program eligibility, and authorize MCOs to begin approved services
  • Coordinate with MEPD staff for enrollment in a Medicaid Waiver program and manage interest lists, ensuring accurate contact information and timely follow-up contacts
  • Interpret policy and coordinate activities related to transitioning individuals from the nursing home to the community, handling continuum of care issues, and assisting with fair hearing requests
  • Handle duties such as completing reports to monitor program operations, researching and resolving errors in enrollment, and making referrals to MEPD for members who lose Supplemental Security Income (SSI) eligibility


Requirements and Qualifications



  • Degree in Social Work, Public Health, Healthcare Administration, or related field
  • At least 2 years of experience in healthcare, social services, or government administration
  • Excellent communication and interpersonal skills, with ability to convey instructions and explanations effectively
  • Proficiency in Microsoft Office, specifically Outlook, Excel, and Word, as well as state software programs like TIERS, SAS, and CSIL
  • Able to manage stress in high-profile situations while maintaining a professional demeanor


Salary Information


The estimated annual salary for this position is $60,000 - $80,000, depending on qualifications and experience. This estimate is based on national averages and may vary depending on location and other factors.
