Job Summary
The Registered Nurse (RN) Case Manager plays a pivotal role in coordinating the care and service of selected patient populations through the acute care episode, transitioning patients across the continuum. Collaborating with interdisciplinary teams, both internal and external to Hendrick Medical Center, this role aims to improve patient care through effective utilization and monitoring of healthcare resources.
Key Responsibilities
- Assumes a leadership role to achieve desired clinical, financial, and resource outcomes.
- Works collaboratively with healthcare professionals to ensure seamless patient care.
- Monitors and evaluates patient progress, making adjustments as needed to optimize care.
- Develops and implements effective care plans, taking into account patient needs and preferences.
Requirements
- Graduate of an accredited School of Nursing.
- 3 years clinical experience as a staff nurse in an acute care hospital, with Charge/Nurse Manager experience beneficial.
- Registered Nurse licensure and certification through Commission for Case Manager (CCM) or American Case Management Association (ACMA) within 2 years of hire.
