Job Overview

Enhabit Home Health & Hospice is seeking a highly skilled Clinical Team Lead to join our team. As a leader in home-based care, we prioritize delivering exceptional patient care and fostering a collaborative work environment.

Estimated Salary: $85,000 per year

This role requires strong leadership skills, with a focus on coordinating home health services to meet the needs of our clients. The ideal candidate will have experience in home health, a good understanding of regulatory guidelines, and proficiency with technology, including EMR platforms.

Key Responsibilities
  • Coordinate home health services to ensure high-quality patient care
  • Review and approve patient information submitted by Licensed Professionals
  • Assist with case conferences, reviews, and approvals
  • Oversight of patient care, staff management, and education
Requirements
  • Graduate of an accredited school of professional nursing and licensed in the state of employment
  • A minimum of two years' nursing experience and one year in home health
  • Skilled competency with technology, general office equipment, and EMR platforms
  • Valid state driver's license and automobile liability insurance
