Job Summary:
Catholic Charities Brooklyn and Queens is seeking an experienced Family Support Specialist to provide comprehensive support services to families and individuals at risk of homelessness. As a Family Support Specialist, you will assess client needs, develop personalized service plans, and connect them with community resources. This is a full-time position that requires a strong background in social work, case management, or a related field.

Responsibilities:
* Assess client needs and develop service plans
* Provide case management services, including crisis intervention and advocacy
* Connect clients with community resources and services
* Maintain accurate and up-to-date client records
* Collaborate with internal and external stakeholders to ensure seamless service delivery

Requirements:
* Master's degree in social work or a related field
* 2+ years of experience in case management or a related field
* Excellent communication and interpersonal skills
* Ability to work in a fast-paced environment and prioritize multiple tasks
