Job Description

The Lower East Side Family Union is seeking a highly motivated and experienced Family Support Coordinator to join our team.

The successful candidate will be responsible for providing case management services to families, with the goal of strengthening family relationships and promoting self-sufficiency. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively with diverse populations.

Key Responsibilities:

  • Provide counseling and case management services to clients, including assessment, service plan development and implementation, and crisis intervention;
  • Maintain frequent and face-to-face contact with clients in their homes;
  • Make visits to schools, hospitals, and other community locations to support clients;
  • Document progress and obstacles in meeting service plan goals;
  • Attend agency-sponsored meetings and trainings;
  • Prepare and submit timely reports to ACS.

Estimated Salary: $70,000 - $85,000 per year.
