Medication Safety Pharmacist

The Medication Safety Pharmacist plays a critical role in enhancing patient safety by improving medication management processes. This position focuses on identifying, evaluating, and mitigating risks related to medication use to reduce potential harm and organizational loss. Key responsibilities include conducting educational programs, monitoring safety activities, and collaborating with various departments to achieve medication safety goals aligned with the objectives of Johns Hopkins Medicine.
Responsibilities
• Conduct educational programs to enhance medication safety knowledge among healthcare professionals
• Monitor safety activities and identify areas for improvement
• Collaborate with pharmacy, nursing, physicians, information systems, risk management, and clinical information teams to achieve medication safety goals
• Develop and implement strategies to mitigate medication-related risks
Requirements
• Bachelor of Science in Pharmacy required
• Master of Science, or a Doctor of Pharmacy Degree from an accredited School of Pharmacy is preferred
• Completion of an American Society of Health System Pharmacists (ASHP) approved residency in hospital pharmacy or a minimum of five (5) years' experience in a hospital pharmacy is required
• Registered or eligible to be registered in the State of Maryland as a Pharmacist
