Ambulatory Care Pharmacist Opportunity

We are seeking a dedicated and experienced Ambulatory Care Pharmacist to join our dynamic healthcare team at Pharmacy Careers.

Key Responsibilities:

  • Conduct comprehensive medication reviews for ambulatory patients to identify, resolve, and prevent medication-related problems.
  • Develop personalized medication action plans and ensure proper medication management and adherence.
  • Collaborate with healthcare providers to optimize medication therapy and enhance patient outcomes.
  • Provide patient counseling and education on medication use, adherence, and potential side effects.
  • Monitor patient progress and adjust medication therapy as needed to achieve optimal therapeutic results.
  • Utilize electronic health records (EHR) to document interventions and track patient outcomes.
  • Participate in the development and implementation of clinical protocols and guidelines for ambulatory care.
  • Stay current with best practices and advancements in ambulatory care pharmacy.
  • Assist with transitions of care to ensure continuity and safety of medication therapy.
  • Engage in quality assurance programs and continuous improvement initiatives to enhance pharmacy services.

Preferred Qualifications:

  • Bachelor's of Pharmacy or PharmD degree from an accredited pharmacy program.
  • Experience in ambulatory care pharmacy.
  • State pharmacy license in good standing.
  • Proficiency in utilizing electronic health record (EHR) systems.
  • Strong analytical and problem-solving skills.
  • Excellent communication and collaboration abilities.
  • Board certification in ambulatory care pharmacy (e.g., BCACP) preferred.
  • Familiarity with chronic disease management and preventive care strategies.
  • Dedication to ongoing professional development and continuous improvement.
