Position Summary:
The Case Manager plays a crucial role in assisting residents of transitional housing in obtaining necessary care and services. This position involves advocating for the needs of residents and facilitating their journey towards permanent housing. The Case Manager collaborates with assigned residents to create comprehensive housing plans and guides them in achieving their income and housing objectives.
The ideal candidate thrives in a collaborative environment with staff and volunteers, demonstrating the ability to respond effectively in urgent situations. This individual possesses strong critical thinking skills to help residents identify actionable steps towards stability. They can work autonomously while ensuring consistent and clear communication with team members. The most suitable candidate is organized, detail-oriented, and adept at establishing connections with community resources, while also encouraging residents to follow through on their commitments. The representative must be energetic, resourceful, and dependable, exhibiting excellent interpersonal skills and a genuine passion for supporting vulnerable populations. This role aims to equip program residents with the necessary tools and support to successfully transition out of homelessness.
Key Responsibilities:
- Manage a caseload of up to 20 residents, conducting weekly interviews with each resident.
- Collaborate with residents to formulate a housing plan that includes goals related to income, health, and housing search, while monitoring progress and providing necessary interventions.
- Maintain precise records in case files and HMIS, documenting interactions with residents, family members, and social agencies.
- Prepare and submit monthly reports and other relevant documentation.
- Coordinate with other program staff regarding resident engagement within the program.
- Assist residents in navigating community systems, including attending appointments and providing hands-on support to overcome obstacles.
- Advocate with referral agencies to ensure residents receive appropriate services.
- Facilitate problem-solving sessions with residents, conducting both group meetings and individual coaching as needed.
- Perform additional duties as assigned by the Director of Ozanam Manor.
- Actively participate in and exemplify SVdP's Culture of Philanthropy, ensuring that SVdP volunteers feel welcomed and valued.

Preferred Qualifications:
- Strong communication skills.
- Professional appearance: Business casual.
- Proficiency in Spanish is preferred.
- Familiarity with psychiatric, medical, and human service delivery systems.
- Commitment to volunteer engagement and appreciation, as well as participation in the Culture of Philanthropy.
Education and Experience:
- Bachelor's degree in a related field is preferred.
- Proficient in standard computer applications (Microsoft Office).
Physical Requirements:
- Ability to stand, walk, and lift up to 25 pounds.
- This position involves frequent direct interaction with staff, volunteers, and clients.
