Matrix Providers is seeking a Pharmaceutical Care Specialist to become part of our skilled team providing healthcare services to Military Service Members and their families.

  • Employment Type: Full-Time
  • Compensation: This position offers a salary, paid bi-weekly OR an hourly wage, paid bi-weekly.
  • Work Schedule: Monday to Friday

Qualifications: The Pharmaceutical Care Specialist must possess the following:

  • Degree: Doctor of Pharmacy (Pharm.D.)
  • Education: Graduate from an institution accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Internship/Residency: Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy, or documentation of relevant education, training, and continuing medical education in clinical pharmacy practice.
  • Experience: As required to meet clinical competency standards specified in the Service-specific credentialing guidelines. A minimum of one (1) year of full-time experience in the last three (3) years as a clinical pharmacist in an outpatient care setting OR completion of an ASHP accredited Ambulatory Care Residency OR possession of Board Certification as a Pharmacotherapy Specialist.
  • Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.

Key Responsibilities:

  • Evaluate patient responses to medication therapy and develop treatment plans based on physician-established diagnoses.
  • Order and assess laboratory tests necessary to evaluate the effects and outcomes of drug therapy.
  • Initiate, adjust, or discontinue medications for ongoing management of chronic conditions in collaboration with the medical team.
  • Monitor and manage pharmacotherapy requiring periodic adjustments due to specific or changing pharmacokinetic characteristics.
  • Administer prescription or non-prescription medications according to established protocols.
  • Assess nutritional needs and order therapeutic nutrition products in the inpatient environment.
  • Review medical and medication histories for drug-related issues and modify drug therapy as needed.
  • Consult with other healthcare professionals regarding patient treatment needs or options.
  • Perform a comprehensive range of pharmacist duties.
  • Review patient profiles during dispensing to closely monitor medication therapy.
  • Ensure prescription instructions, dosage, medication interactions, allergy information, and other relevant data are appropriate for individual patients.
  • Guarantee the accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Provide training to clinical staff on new medications, appropriate usage, and other relevant topics.
  • Collaborate with specialty practitioners to ensure optimal patient pharmaceutical care.
  • Document significant medication interactions and pharmacy communications with prescribers. Conduct Adverse Drug Reaction (ADR) reporting as required.
  • Facilitate drug utilization reviews as necessary.
