Job Summary

We are seeking a skilled Case Manager to provide case management/community services for the clients of The Salvation Army Harbor Light. This is an exciting opportunity to make a difference in the lives of individuals and families in need.

Key Responsibilities
  • Conduct educational lectures and workshops to support client learning and development
  • Maintain accurate and up-to-date case records in accordance with Harbor Light standards
  • Provide case management services to treatment clients, including assessment, planning, and coordination of services
  • Assist clinical staff with various tasks as assigned, such as preparing packets and addressing non-clinical issues
  • Participate in the review of clinical issues, activities, and program policies and procedures
  • Attend all clinical staff meetings and team conferences
  • Participate in educational courses, seminars, workshops, and in-service meetings to enhance treatment skills
Requirements
  • A bachelor's degree in social work or an equivalent comparable human services field
  • Experience in case management, preferably in a mental health or substance abuse setting
  • Excellent communication and teamwork skills
  • Strong organizational skills
  • Ability to determine needs and work with multi-problem individuals
  • Ability to develop rapport, trust, and confidence in client relationships
Preferred Qualifications
  • MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 2 weeks of hire date
  • Valid Michigan Chauffeur License and approved by The Salvation Army MVR
