Job Summary

Talent4Health is seeking a skilled Nurse Case Manager to oversee the daily operations of our case management department. As a key member of our team, you will be responsible for supervising the delivery of initial psychosocial assessments and follow-up assessments, maintaining current knowledge of community resources, and ensuring the accurate and timely implementation of new and existing programs.

Key Responsibilities
  • Supervise Case Management Team: Oversee the daily operations of the case management department, including the supervision of case managers and the delivery of initial and follow-up assessments.
  • Maintain Community Resources: Stay up-to-date on community resources and communicate with hospital case managers to ensure the supply of resources materials.
  • Implement Programs: Ensure the accurate and timely implementation of new and existing programs to achieve departmental and organizational goals and objectives.
  • Develop Staff: Engage and evaluate the competency-based performance of staff members, develop educational curriculums and tools, and provide immediate feedback to staff members.
  • Collaborate with Stakeholders: Partner with key stakeholders along the continuum of care to provide daily review for complex or catastrophic cases and treatment plans.
Requirements
  • Experience: Minimum of three years of clinical experience, with a focus on hospital-based case management.
  • Skills: Epic and Careport experience, acute care case management leadership experience, discharge planning and utilization management experience, and case management experience.
  • Licenses and Certifications: Full license to practice as a Registered Nurse in the State of Michigan or a Full License to practice as a Social Worker in the State of Michigan.
  • Education: Bachelor's degree in nursing, healthcare management, or a related healthcare field.
