Job Title: Case Manager

We are seeking a highly motivated and compassionate Case Manager to join our team at the Ruth Ellis Center. As a Case Manager, you will provide direct youth-centered and strengths-based case management to LGBTQ+ youth and young adults ages 18 to 30 in our Supportive Housing Programs.

Job Summary:

The Case Manager will provide services both at Ruth Ellis Center facilities and in the community, including coffee shops, home visits, libraries, etc. The ideal candidate will have a passion for advocacy and housing justice, and will be able to take a person-centered approach, show empathy, establish clear boundaries, and promote empowerment.

Responsibilities:
  1. Support at least 10 youth in the supportive housing programs, which requires building authentic, caring, and affirming relationships with REC youth, participating in weekly case review meetings and supervision, and providing a trauma-informed, anti-racist, and anti-oppression approach to youth, colleagues, and self.
  2. Develop self-care practices, including maintaining appropriate boundaries and using time off to support rest and rejuvenation.
  3. Complete intakes and assessments with youth, document youths' housing histories, and help eligible youth navigate the coordinated entry process to be successfully referred to REC's supportive housing programs.
  4. Accompany youth to community resources such as Coordinated Assessment Model intakes, apartment searching, applying for benefits, medical appointments, and other appointments to assist with enrollment and/or advocacy as needed.
  5. Support the health needs of youth experiencing co-occurring behavioral health and substance use disorders.
  6. Support youth in identifying their strengths and using them to set and achieve goals together.
  7. Provide youth flexible, responsive case management that addresses their variety of needs, including accessing shelter, obtaining identification, enrolling in high school, GED programs, and college, navigating financial aid, accessing public benefits, connecting to health, mental health, and substance abuse care, accessing childcare, connecting to employment and permanent housing, and other supports.
  8. Check in with youth at least one a month, and more frequently as needed, about progress on their goals and how REC and its partner agencies can support them.
  9. Coordinate and communicate with clients, service providers, and family members to ensure optimal utilization of services and adequate provision of needed care.
  10. Seek and pick up furniture for newly housed youth.
  11. Support the development of REC Center service team through active communication, participating in case conferencing, and sharing knowledge and resources.
  12. Assist with other duties as assigned.
Requirements:
  • Two (2) years' experience in homelessness recovery services, HIV services, substance use disorder, behavioral/mental health, and/or medical field.
  • Ability to take a person-centered approach, show empathy, establish clear boundaries, and promote empowerment.
  • Ability to de-escalate crisis situations and assist individuals in crisis achieve safety.
  • Ability to meet deadlines and manage multiple ongoing tasks. Ability to prioritize competing demands for time and attention to ensure clients are able to achieve their goals.
  • LGBTQ (Lesbian, Gay, Bisexual, Transgender, and Questioning) cultural and diversity competency.
  • Familiarity with the Ruth Ellis Center's principals of work (trauma informed care, harm reduction, restorative justice, and positive youth development).
  • SOAR (SSI/SSDI Outreach, Access, and Recovery) trained or willingness to complete SOAR certification within a mutually agreed timeframe.
  • Experience with Microsoft Office and Google Suite Software.

Pay Range: $18 - $23 per hour
