MetaSense, Inc. is seeking a skilled nurse to fill a contract position in case management. The ideal candidate will have a strong background in nursing and case management, with a focus on hospital-based care.

Key Responsibilities:

  • Provide case management services to patients in a hospital setting
  • Develop and implement individualized care plans to meet patient needs
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and maintain relationships with patients, families, and healthcare providers

Requirements:

  • Bachelor's degree in nursing or related field
  • Minimum of three years clinical experience; hospital-based case management preferred
  • Working knowledge of case management principles and practices
  • Excellent communication and interpersonal skills
  • Strong analytical and problem-solving skills
  • Ability to work independently and as part of a team

Preferred Qualifications:

  • Certification in case management (CCM) by the Commission for Case Management Certification (CCMC)
  • Experience with Epic and Careport systems
  • Current acute care case management leadership experience (2 year minimum required)
  • Inpatient hospital case management experience
  • Discharge planning and utilization management experience
