Position: Healthcare Case CoordinatorWe are seeking a Healthcare Case Coordinator who is detail-oriented, compassionate, and an excellent collaborator. In this role, you will be instrumental in improving the quality of our clients' experiences by leveraging your understanding of the healthcare system to guarantee they receive thorough care. Your duties will encompass:
- Educating and empowering clients and their families to navigate health challenges effectively.
- Connecting clients with community resources and essential support systems.
- Coordinating client care in alignment with physicians' recommendations.
- Overseeing treatment plans and maintaining comprehensive client records.
- Collaborating with healthcare professionals to ensure every client receives the care they are entitled to.
- Advocating for clients during significant medical appointments.
- Assisting with hospital discharges to guarantee clients return to a secure environment.
- Developing treatment programs and monitoring clients' progress diligently.
Preferred Qualifications:
We favor candidates with a degree in Nursing, Case Management, Physical Therapy, or related disciplines, along with experience in medical management and knowledge of HIPAA regulations.
Compensation Structure:
This role offers a flexible contract with competitive remuneration based on skills and experience.
