As a Case Management Specialist, you will be responsible for providing high-quality patient care and case management services. You will work closely with our team to develop and implement effective care plans, ensuring the best possible outcomes for our patients.
Requirements- MUST HAVE Epic and Careport experience
- MUST HAVE current acute care case management leadership experience (2 year minimum required)
- MUST HAVE inpatient hospital case management experience
- MUST HAVE discharge planning and utilization management experience
- MUST HAVE case management experience
- MUST HAVE Full License to practice as a Registered Nurse in the State of Michigan or Full License (LMSW) to practice as a Social Worker in the State of Michigan
- MUST HAVE Certification in Case Management (CCM) by the Commission for Case Management
- MUST HAVE Bachelor's degree in nursing, healthcare management or related health care field required or a Masters of Social Work required
