Role Summary

The Patient Care Coordinator plays a vital role in managing and organizing care for patients within their assigned unit. This position involves regular communication with family members and community support systems regarding the patient’s treatment journey.

Key Responsibilities
  • Conduct initial meetings with patients within one business day to complete necessary documentation and assess any required releases of information.
  • Identify and communicate the needs of referral sources and professionals during hospitalization to the Lead Coordinator.
  • Make regular outreach calls to collateral contacts for each patient, ensuring at least one call is made weekly.
  • Facilitate ongoing communication regarding treatment progress, discharge plans, and necessary documentation transfers to external parties.
  • Educate patients and families about discharge planning options based on individual circumstances and available resources.
  • Document all findings, discharge plans, and actions taken in accordance with departmental protocols; prepare necessary reports and maintain accurate records.
  • Engage in multidisciplinary team meetings to discuss patient care planning and discharge processes, promoting effective communication and problem-solving.
  • Uphold the organization's policies regarding Family Engagement and report any challenges to the Treatment Team.
  • May receive training and be assigned to perform Patient Advocate duties as needed.
Qualifications
  • Education & Licensure: Bachelor's Degree in Social Work or a related mental health field from an accredited institution is preferred.
  • Experience: At least one year of relevant experience, particularly in a hospital setting, is strongly preferred.
  • Additional Skills: Completion of CPI training within the first month of employment is required, along with passing a competency exam for the role. Flexibility in work hours and willingness to work overtime may be necessary.
