About This Role

We are seeking a highly skilled Care Coordinator to join our team at Commonwealth Care Alliance. As a Care Coordinator, you will play a critical role in ensuring our members receive high-quality, patient-centered care.

Key Responsibilities
  • Member Engagement: Develop and maintain strong relationships with our members, understanding their unique needs and preferences.
  • Care Planning: Collaborate with members, healthcare providers, and other stakeholders to create personalized care plans that address their physical, emotional, and social needs.
  • Health Navigation: Provide education and support to help members navigate the healthcare system, access necessary services, and make informed decisions about their care.
  • Communication: Serve as a primary point of contact for members, responding to their questions and concerns in a timely and compassionate manner.
  • Team Collaboration: Work closely with our care team, including nurses, social workers, and other professionals, to ensure seamless coordination of care.
  • Documentation: Maintain accurate and up-to-date records of member interactions, care plans, and other relevant information.
  • Quality Improvement: Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of strategies to enhance member outcomes.
Requirements
  • Education: High school diploma or equivalent required; associate's or bachelor's degree preferred.
  • Experience: 1-2 years of experience in a healthcare setting, with a focus on patient care and navigation.
  • Clinical License/Certification: LPN, medical assistant, or CHW certification preferred.
  • Language Skills: English required; bilingual preferred.
