Job Summary

We are seeking a highly skilled Addiction Medicine Case Manager to join our team at Kaiser Permanente. As a key member of our Chemical Dependency department, you will provide Chemical Dependency services and psychotherapy to adults, adolescents, significant others of CD patients, and families under indirect supervision.

Key Responsibilities
  • Answer and triage patient calls for information, treatment services, and general CD advice.
  • Respond to and appropriately handle crisis calls.
  • Perform higher-level assessments, including non-CD and MH diagnoses, danger to self or others, and non-CD crisis evaluations.
  • Develop and present treatment plans to the CDRP/S treatment team for recommendations, revisions, and approval.
  • Participate in clinical case conferences and conduct chemical dependency-focused educational lectures.
  • Provide general chemical dependency counseling, including individual and group counseling services.
  • Provide Adolescent, Significant Other, Family, and Multi-Family therapy as assigned.
  • Provide case management services, including resource and referral coordination.
  • Mentor CDRP/S Counselors I and II, student interns, licensure candidates, and Alcohol or Drug Certificate candidates.
Requirements
  • Minimum of two (2) years post-graduate relevant supervised addiction medicine therapy experience within the last five (5) years.
  • Masters Degree in Social Work, Psychology, Sociology, or other field related to behavioral health.
  • Licensed Professional Clinical Counselor (California) or Licensed Marriage and Family Therapist (California) or Licensed Clinical Social Worker (California).
Preferred Qualifications
  • Spanish proficiency.
  • Experience working with chronic addiction patients.
  • Ability to travel to patient care locations two days a week.
Work Environment

This is a full-time position with a 40-hour workweek. The work schedule is Monday through Friday, 12:01 AM to 11:59 PM. The worker location is flexible, with the option to work from home. However, the worker must align with Kaiser Permanente's Authorized States policy.
