Job Summary:

As a key member of our team at Kaiser Permanente, the Addiction Medicine Case Manager will provide comprehensive Chemical Dependency services and psychotherapy to adults, adolescents, significant others of CD patients, and families under indirect supervision.

Essential Responsibilities:

Respond to patient calls for information, treatment services, and general CD advice, handling crisis calls with care and professionalism.

Conduct higher-level assessments, including evaluations for non-CD and MH diagnoses, danger to self or others, and non-CD crisis evaluations, to ensure accurate treatment plans.

Develop and present treatment plans to the CDRP/S treatment team for recommendations, revisions, and approval, ensuring seamless collaboration and effective care.

Participate in clinical case conferences to discuss patient progress and treatment strategies, promoting a culture of continuous learning and improvement.

Provide chemical dependency-focused educational lectures to patients and their families, empowering them with knowledge and skills to manage their addiction.

Offer general chemical dependency counseling, including individual and group counseling services, to support patients in their recovery journey.

Provide Adolescent, Significant Other, Family, and Multi-Family therapy as assigned, addressing the unique needs of each patient population.

Offer case management services, including resource and referral coordination, to ensure patients receive comprehensive support and care.

May perform mentor functions and clinical direction to CDRP/S Counselors I and II, student interns, licensure candidates, and Alcohol or Drug Certificate candidates, fostering growth and development within the team.

Basic Qualifications:

Minimum of two (2) years post-graduate relevant supervised addiction medicine therapy experience within the last five (5) years required.

Master's Degree in Social Work, Psychology, Sociology, or other field related to behavioral health.

Licensed Professional Clinical Counselor (California) OR Licensed Marriage and Family Therapist (California) OR Licensed Clinical Social Worker (California).
