Job Summary

Key Responsibilities
  • Conduct psychological screenings and develop individualized plans of care to promote resident adjustment to rehabilitation or long-term care.
  • Collaborate with families, business office, and Medicare/Medicaid to facilitate applications and qualification for sub-acute/long-term financial assistance.
  • Schedule and lead interdisciplinary team meetings to plan care and resolve concerns.
  • Provide guidance and counseling to residents and their families on diverse issues, including grief and loss, acceptance, and coping with changes in living arrangements.
  • Coordinate discharge services, including planning and acquisition of appropriate follow-up care.
Requirements
  • 1-3 years of long-term experience in a healthcare setting.
  • Master's Degree in Social Work, Psychology, or a related field.
  • Social Work License.
