Key Responsibilities- Referral Management: Manage and accept referrals, ensuring timely and accurate processing.
- Communication: Initiate and maintain effective communication with clinical and operations teams to facilitate individual placement and services.
- Intake Processing: Collect, verify, record, and process intake demographics, insurance/payment, and referral information.
- File Management: Create files for new individuals and ensure intake paperwork has been completed/requested.
- Reporting: Complete all reporting requirements consistent with corporate and regulatory standards.
- Problem-Solving: Identify and resolve issues that may prevent successful documentation/entry.
- Education: Bachelor's Degree in Human Services or related field.
- Experience: Two years of experience preferred, or an equivalent combination of education and experience.
- Skills: Self-motivated, detail-oriented, excellent communication and organizational skills, and a commitment to Sevita's mission and values.
