The Healthcare Intake Specialist plays a crucial role in ensuring all necessary paperwork and approvals are in place, and coordinating the smooth transition of patients into our care. This position requires excellent communication and organizational skills to work effectively with patients, families, physicians, and healthcare professionals. Key Responsibilities:

  • Receive and process referrals from hospitals, physicians, and other healthcare facilities.
  • Collect and verify patient information, including medical history, insurance coverage, and eligibility for home healthcare services.
  • Serve as the primary point of contact for new patients and their families, providing clear information about the home healthcare process.
  • Work with clinical staff to schedule initial assessments and match patients with appropriate care providers.
  • Obtain necessary authorizations and approvals from insurance companies, Medicare, or Medicaid.
  • Ensure compliance with federal, state, and local regulations, including HIPAA requirements.
  • Maintain accurate records of patient intake and admission processes in our electronic health records (EHR) system.
  • Communicate with external healthcare providers to coordinate patient care plans.
  • Collaborate with the Director of Nursing and other team members to ensure seamless patient transitions into care.
Qualifications:
  • High school diploma or equivalent required; associates or bachelors degree in healthcare administration or a related field is preferred.
  • Minimum of 1-2 years of experience in a healthcare setting, preferably in intake coordination, admissions, or case management.
  • Knowledge of home healthcare services, Medicare/Medicaid guidelines, and insurance verification processes.
  • Strong interpersonal and communication skills, with the ability to effectively communicate with patients, families, and healthcare professionals.
  • Excellent organizational skills and attention to detail.
  • Ability to multitask and manage competing priorities in a fast-paced environment.
  • Proficiency in electronic health records (EHR) systems and Microsoft Office (Word, Excel, Outlook).
  • Bilingual (English/Spanish) is a plus.
