Job Summary

We are seeking a skilled Case Manager to join our team at UMass Memorial Health. The successful candidate will be responsible for providing comprehensive case management services to clients, including assessment, planning, and coordination of care. This is a rewarding opportunity to make a positive impact on the lives of our patients and their families.

Key Responsibilities
  • Conduct comprehensive case management intake assessments and develop plans with clients to obtain necessary services.
  • Complete initial behavioral health intake documentation as required by program, payor, and regulatory bodies.
  • Collaborate with the integrated team to coordinate delivery of appropriate behavioral health and other support services.
  • Provide ongoing case management services to assist clients with obtaining entitlements and benefits, including SSI, EAEDC, MassHealth, and other health insurance needs.
  • Assist clients in accessing transportation through existing resources or accessing new resources.
  • Ensure aftercare needs of clients are met through provision of appropriate referrals for community-based services.
  • Provide outreach to community-based residents as designated.
  • Dispatch into the community with a clinician to support AMCI and YMCI programs as needed.
Requirements
  • High school diploma/GED.
  • Satisfactory CORI background check.
  • Ability to develop a working knowledge of CHL's service lines.
  • Excellent oral communication and computer skills.
  • Ability to multitask and prioritize tasks effectively.
  • Personal and professional accountability, self-management, and initiative.
