Caregiver Specialist

Ohio's Hospice is seeking a compassionate and dedicated Personal Care Specialist to join our team. As a Personal Care Specialist, you will play a vital role in providing exceptional care and support to patients and their families.
Key Responsibilities- Review patient plans of care to ensure accurate and timely delivery of services.
- Provide personal care services, including bathing, dressing, oral hygiene, and meal preparation, as outlined in the patient's plan of care.
- Maintain a clean and comfortable environment for patients, adjusting lighting and room temperature as needed.
- Report vital signs and other equipment readings to the RN in a timely and accurate manner.
- Participate in interdisciplinary team meetings and communicate with the RN regarding patient changes or updates.
- Document activities and interventions in the electronic health record.
- Follow individualized plans of care and document comprehensive observations, interventions, and communication with the RN.
- Recognize and report changes in patient mental or physical status to the RN promptly.
- High School Diploma or GED
- Certified State Tested Nursing Assistant with the State of Ohio without any board actions
- One year of experience preferred
- CHPNA certification required or after completion of required hours for certification
- Computer skills sufficient to properly document services and care
- Valid Driver's License with a safe driving record
- State Minimum Automobile Insurance Coverage
