About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in need of end-of-life care.

Responsibilities
  • Provide comprehensive patient assessments and develop individualized care plans
  • Collaborate with the patient's physician, IDG, and family to ensure seamless care coordination
  • Supervise hospice aides and provide education and support to patients and families
  • Manage admissions, routine visits, recertifications, discharges, and deaths
Requirements
  • Active RN license in the state of Minnesota
  • Basic Cardiac Life Support (BCLS) certification
  • CPR certification (or willingness to obtain within 6 months)
  • Valid driver's license and use of an insured personal vehicle
