Key Responsibilities:

  • Lead and mentor social services staff while contributing to community health and welfare planning.
  • Ensure residents receive essential medically-related social services that address their physical, mental, and psychosocial needs.
  • Document each resident's condition upon entry and coordinate the discharge planning process effectively.
  • Conduct thorough evaluations and maintain comprehensive care plans tailored to each resident's unique circumstances.
  • Act as a liaison among residents, their families, external agencies, and facility administration to uphold residents' rights and ensure their needs are met.

Qualifications:

  • A Bachelor's degree in social work or a related human services field is required; a Master's degree is preferred.
  • Must hold a valid license in accordance with New Jersey state regulations (LCSW).
  • Possess strong interpersonal communication skills and a thorough understanding of relevant governmental regulations.
  • Ability to engage with community resources to facilitate effective discharge planning.
  • Comfortable working independently as well as collaboratively within a team environment.
