Job Description

V-Tech Solutions, Inc. is seeking a highly detail-oriented and passionate Care Manager I (Bilingual) to support our client in the D.C. area. The ideal candidate will have a Master's Degree and at least two years of experience in healthcare or related fields, with a strong background in care management and coordination.

Key Responsibilities:
  • Manage a caseload of members with moderate to complex medical and psychosocial needs.
  • Develop, monitor, and evaluate treatment plans to ensure optimal care outcomes.
  • Communicate effectively with members, caregivers, and healthcare providers to coordinate care and services.
  • Identify and address gaps in care, including over- and under-utilization of services.
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans.
  • Document interactions and care coordination activities in a timely and accurate manner.
Requirements:
  • Master's Degree in a related field.
  • At least two years of experience in healthcare or related fields.
  • Licensed in DC as a Licensed Graduate Social Worker (LGSW).
  • Certified Case Manager (CCM) certification preferred.
  • Strong communication and interpersonal skills.
  • Ability to work in a fast-paced environment and meet deadlines.
