Job Title: Clinical Social WorkerJob Summary

Patient Centered Services LLC is seeking a skilled Clinical Social Worker to join our team. As a key member of our healthcare team, you will provide psychosocial care and support to patients and their families in home health and community settings.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to determine patients' needs and develop individualized care plans.
  • Collaborate with interdisciplinary healthcare teams to address the mental, social, and emotional needs of patients.
  • Provide case management services, including coordination with community resources and support programs.
  • Document patient interactions and maintain accurate records in our electronic medical record system.
  • Facilitate access to social services and community resources to ensure patients receive appropriate assistance.
Requirements
  • Bachelor's degree in Social Work (BSW) or related human services field from an accredited institution.
  • Minimum of 1-2 years of experience in a social work or human services role, preferably in healthcare or home health settings.
  • Strong understanding of social work principles and ability to collaborate in a team environment.
  • Effective communication skills, both verbal and written.
Preferred Qualifications
  • Masters degree in Social Work (MSW) from an accredited program (in progress or pending licensure).
  • Experience working with diverse populations, such as pediatric or geriatric patients.
  • Multilingual abilities (especially English/Spanish) are preferred but not required.
