Job Overview

The Medical Coordinator plays a vital role in delivering high-quality medical care to patients experiencing homelessness in our recuperative care setting.

Responsibilities
  • Client Care:
    • Conduct initial intake assessments to establish appropriate care plans
    • Assess patient health status, determine insurance status, and make follow-up appointments with providers
    • Establish and maintain care plans, set weekly goals, and verify goal completion
    • Conduct regular checks to observe patient wellbeing and ensure no change in medical conditions
    • Link patients to medical services, coordinate home health visits, and follow-up on medical appointments
    • Promote health literacy through patient education on illnesses, medications, and preventative measures
    • Arrange transportation for patients to and from medical appointments
    • Report emergency situations to Supervisor/Manager and document incidents
  • Administrative/Record Keeping:
    • Enter patient information and case notes into electronic health records
    • Complete and update medication, vitals, progress notes, and care plans
    • Perform administrative tasks, including answering phones and filing paperwork
    • Prepare patient reports for billing
    • Maintain proper inventory of medical supplies and medications
  • Compliance/Safety:
    • Maintain confidentiality in accordance with HIPAA guidelines, Illumination Foundation policies, and DHS standards
    • Provide patients with compliance forms upon admission
    • Utilize infection control measures according to OSHA standards
    • Conduct medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste
    • Maintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and medication storage room
Qualifications
  • Current and non-restricted LVN license in California
  • 2 years of relevant experience
  • High School Diploma or equivalent
  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets
  • Valid CA Driver's License
