Clinical Care Coordinator

As a Clinical Care Coordinator at the University of California, Irvine, you will play a vital role in ensuring the highest quality patient care. Your primary responsibility will be to assess and reassess patients' needs, setting priorities and implementing care plans that promote self-care outcomes.
Collaborating with an interdisciplinary team, you will facilitate the delivery of individualized, safe, and cost-effective patient care, as well as coordinate transitions of care. Your advocacy for patient self-determination and cultural competence will be essential in evaluations and planning.
You will also monitor and coordinate resource utilization, evaluating services provided and performing utilization review functions using medical staff-approved decision support criteria.
Key Responsibilities:
- Conduct high-level assessments and reassessments to set priorities and implement care plans
- Collaborate with an interdisciplinary team to facilitate patient care
- Advocate for patient self-determination and cultural competence
- Monitor and coordinate resource utilization
- Perform utilization review functions using medical staff-approved decision support criteria
Requirements:
Minimum 5 years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a BSN.
Familiarity with Joint Commission, CMS, CDPH, and CCS requirements.
Excellent written and verbal communication skills in English.
Current RN License.
Computer literacy and proficiency.
Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload.
Ability to facilitate and lead interdisciplinary rounds.
Ability to establish and maintain effective working relationships across the Health System.
Preferred Qualifications:
Knowledge of University policies and procedures.
Knowledge of California Children's Services benefits authorization process (CCS).
CCM, ACM, or Case Management credentialing.
Bilingual skills.
