Job Summary

We are seeking a highly skilled and compassionate Licensed Therapist to join our team at LifeLong Medical Care. As a key member of our behavioral health department, you will provide comprehensive psychosocial assessments, mental health diagnoses, and treatment plans for patients with a range of issues.

Responsibilities
  • Conduct comprehensive psychosocial assessments and mental health diagnoses for patients with issues such as depression, anxiety, trauma, grief/loss, addiction, domestic violence, behavioral health problems, and psychosocial stressors.
  • Develop and implement short-term counseling plans using appropriate modalities.
  • Provide short-term case management services for patients participating in treatment.
  • Assist with crisis intervention and triage as needed, including assessment, de-escalation, and notifying the public mental health crisis response team if necessary.
  • Work with team members to provide follow-up assessment and prevention planning after any crisis, including follow-up with outside agencies.
  • Maintain clinical and administrative records in accordance with contractual and billing requirements.
  • Participate in weekly clinical meetings and promote integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.
  • Participate in quality improvement efforts related to behavioral health and integrated models of care.
  • Consult informally with providers as needed.
  • Follow all State of California mandated reporting requirements.
  • May serve as supervisor for psychology or MSW interns and may provide ASW or AMFT or APCC clinical supervision.
Requirements
  • Current and valid California LCSW, LMFT, LPCC license, or ACSW or AMFT registration with the California Board of Behavioral Sciences.
  • Knowledge and experience providing mental health services to disadvantaged populations.
  • Familiarity with evidence-based practices for the treatment of depression and other mental disorders.
  • Proficient in Microsoft office.
Preferred Qualifications
  • Supervisory experience.
  • Bilingual English/Spanish speaking.
  • Epic and or medical software systems knowledge.
  • Community Healthcare experience.
