Job Summary

The Hospice & Palliative Care Charlotte Region is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Hospice House. As a LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families in a homecare or facility setting.

Key Responsibilities

  • Delivery of Patient Care
    • Participate in the assessment of the patient's physical and mental health, including their reaction to illness and treatment regimens.
    • Record and report the results of the nursing assessment.
    • Implement the Plan of Care developed by the registered nurse and/or prescribed by any person authorized by State law, by performing tasks assigned or delegated by and performed under the supervision or under orders or directions of a registered nurse, licensed physician or any other person authorized by State law to provide supervision.
    • Facilitate patient and caregiver independence to the extent possible.
    • Participate in the teaching and counseling of patients as assigned by a registered nurse, physician or other qualified professional authorized by State law.
  • Interdisciplinary Group
    • Attend and participate in the interdisciplinary group conferences [IDG] and special care conferences.
    • Seek and utilize the appropriate resources for the referral process, as directed by the RN.
    • Participate in identifying specific patient/caregiver needs and making referrals to appropriate team members.
  • Documentation
    • Ensure accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.
  • Professional and Personal Accountability
    • Demonstrate competency in knowledge and skills within scope of practice.
    • Ensure that VIA Health Partners professional reputation is maintained and projected.
    • Assume responsibility for professional development and staying abreast of current trends in the healthcare field.
    • Incorporate new information and methods into practice.
    • Maintain current professional licensure, CPR certification and CEU requirements.
    • Exhibit time management skills.
    • Able to plan workday while remaining able to respond appropriately to unforeseen obstacles.

Qualifications

  • A Diploma from a Board approved nursing program designed to prepare a person for practical nurse licensure is required;
  • A current, active, and unrestricted compact (multistate) license as a Licensed Practical Nurse (LPN) is required.
  • The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state's practice laws and discipline.
  • Current CPR required; CHPN preferred.
  • Minimum 1-year related experience for LDHH and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
  • Computer proficiency is required.
