Key Responsibilities
  • Patient Care Delivery
  • Assist with personal care tasks, including:
    • Bathing and grooming assistance.
    • Mobility support and body mechanics.
    • Meal preparation and feeding assistance.
    • Elimination support and specimen collection.
    • Safety measures and infection control practices.
    • Vital signs monitoring and basic medical procedures.
    • Maintain a clean and safe environment for patients and families.
Interdisciplinary Collaboration
  • Participate in team meetings and care conferences.
  • Build effective relationships with team members.
  • Identify patient needs and refer to appropriate professionals.
Documentation and Accountability
  • Ensure accurate and timely clinical documentation.
  • Maintain professional knowledge and skills.

Qualifications

Required Qualifications
  • High School diploma or equivalent.
  • Certified Nursing Assistant certification in good standing.
  • Minimum of 1 year of relevant experience, preferably in home care or with terminally ill patients.
