Care Management Role

The Fortify Care Navigator LPN is a key member of the Care Management department at Children's Hospital of the King's Daughters. This role involves providing care coordination for emerging risk patients, utilizing skills and knowledge to support patient care.

Responsibilities
  • Initiate and complete quarterly telephonic outreach to patients.
  • Complete health risk assessments, social needs assessments, and individualized plans of care.
  • Connect patients to resources for social needs and complete care gaps.
  • Work with interdisciplinary teams to improve patient, family, and community outcomes.
  • Electronically document care according to industry and institutional standards.

Requirements
  • 3+ years of professional healthcare experience.
  • Previous experience working remotely and with children, families, and community partners.
  • Proficiency with computers and documentation in the electronic health record.

About the Role
This is a remote position that requires a home office. The Fortify Care Navigator will work collaboratively with other care team members and care providers to help patients achieve optimal health outcomes.
