Oncology Community Health Advocate

At Sentara, we are seeking a dedicated Community Health Worker to join our Cancer Services Team.
The ideal candidate will work collaboratively with nurse navigators and medical social workers to provide comprehensive care coordination for cancer patients in Hampton Roads.
Key responsibilities include engaging patients in interventions to improve self-management skills and quality of life, as well as acting as a patient advocate and healthcare navigator to assist with resource allocation and management.
We welcome bilingual candidates who are experienced in community-based settings, human services, oncology, and care coordination.
