Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our Cancer Services Team at Sentara Norfolk General Hospital. As a key member of our team, you will work collaboratively with healthcare professionals to provide comprehensive and coordinated care to patients with chronic and complex illnesses.

Responsibilities
  • Engage patients and families in interventions to improve self-management skills and quality of life.
  • Provide education and behavior modification strategies to support patients and families.
  • Act as a patient advocate and healthcare navigator to assist with resource allocation and management.
Requirements
  • High School Diploma or Equivalent
  • Valid Driver's License
  • Bilingual in English and Spanish is preferred
  • Experience in community-based settings, human services, oncology, and care coordination is preferred
