Position Overview:

The Clinical Pharmacist in the Intensive Care Unit (ICU) plays a vital role in delivering pharmaceutical care within a multidisciplinary healthcare team. This position is essential for ensuring safe and effective medication management for patients in critical condition.

Key Responsibilities:
  • Conduct thorough reviews of medication orders for appropriateness and provide final verification.
  • Evaluate the effectiveness, safety, and cost-effectiveness of prescribed medications, collaborating with the healthcare team to optimize patient therapy.
  • Offer comprehensive drug information regarding medication usage, dosages, side effects, and formulations to healthcare professionals and patients.
  • Monitor both inpatient and outpatient medication profiles for potential drug interactions and therapeutic duplications.
  • Engage in transitions of care and assist in resolving medication access issues for patients.
  • Participate in quality improvement initiatives, including medication use evaluations and documentation of clinical interventions.
  • Contribute to pharmacy and medical literature and engage in research activities as applicable.
  • Provide education and training to pharmacy staff, residents, and other healthcare providers.
  • Oversee the compounding, labeling, and packaging of pharmaceuticals, ensuring compliance with safety standards.
  • Respond promptly to emergency situations, including code and trauma alerts.
  • Assist in maintaining appropriate drug inventory levels and ensuring the security of controlled substances.
  • Contribute to the development and ongoing assessment of electronic order entry systems.
  • Attend relevant committee meetings and participate in continuing education opportunities to enhance professional development.
  • Perform additional duties and special projects as assigned.

Qualifications:
  • Minimum of one year of hospital pharmacy experience preferred.
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Completion of a pediatric clinical pharmacy residency is preferred.
  • Current Texas Pharmacist license is required.
  • Basic Life Support (BLS) certification and Pediatric Advanced Life Support (PALS) certification are required within specified timeframes.
  • Board of Pharmacy Specialties certification is preferred.
