Inpatient Clinical Pharmacist

The Clinical Pharmacist in the Intensive Care Unit (ICU) plays a vital role in delivering pharmaceutical care within a multidisciplinary healthcare team. This position is essential for ensuring safe and effective medication management for patients in critical condition.
Key Responsibilities:
- Conduct thorough reviews of medication orders for appropriateness and provide final verification.
- Evaluate the effectiveness, safety, and cost-effectiveness of prescribed medications, collaborating with the healthcare team to optimize patient therapy.
- Offer comprehensive drug information regarding medication usage, dosages, side effects, and formulations to healthcare professionals and patients.
- Monitor both inpatient and outpatient medication profiles for potential drug interactions and therapeutic duplications.
- Engage in transitions of care and assist in resolving medication access issues for patients.
- Participate in quality improvement initiatives, including medication use evaluations and documentation of clinical interventions.
- Contribute to pharmacy and medical literature and engage in research activities as applicable.
- Provide education and training to pharmacy staff, residents, and other healthcare providers.
- Oversee the compounding, labeling, and packaging of pharmaceuticals, ensuring compliance with safety standards.
- Respond promptly to emergency situations, including code and trauma alerts.
- Assist in maintaining appropriate drug inventory levels and ensuring the security of controlled substances.
- Contribute to the development and ongoing assessment of electronic order entry systems.
- Attend relevant committee meetings and participate in continuing education opportunities to enhance professional development.
- Perform additional duties and special projects as assigned.
Qualifications:
- Minimum of one year of hospital pharmacy experience preferred.
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Completion of a pediatric clinical pharmacy residency is preferred.
- Current Texas Pharmacist license is required.
- Basic Life Support (BLS) certification and Pediatric Advanced Life Support (PALS) certification are required within specified timeframes.
- Board of Pharmacy Specialties certification is preferred.
