Key Responsibilities:
- Conduct thorough reviews of medication orders for appropriateness and ensure accurate final verification.
- Evaluate the effectiveness, safety, and cost-effectiveness of prescribed medications, collaborating with the healthcare team to optimize patient therapy.
- Provide essential drug information regarding medication usage, potential side effects, and composition to healthcare professionals and families.
- Monitor patient profiles for drug interactions and ensure safe medication practices.
- Engage in quality improvement initiatives, including medication use evaluations and documentation of clinical interventions.
- Contribute to pharmacy and medical literature and participate in relevant research activities.
- Supervise the preparation and packaging of medications, ensuring compliance with safety standards.
- Respond to emergency situations as required, providing critical support during code and trauma events.
- Assist in maintaining the pharmacy inventory and ensuring the security of controlled substances.
- Participate in the development and review of electronic order entry systems.
- Engage in ongoing professional development through education and training programs.
- Perform additional duties and special projects as needed.
Qualifications:
- Minimum of one year of hospital pharmacy experience preferred.
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
- Completion of a pediatric clinical pharmacy residency is preferred.
- Current Texas Pharmacist license is required.
- Basic Life Support (BLS) certification and Pediatric Advanced Life Support (PALS) certification are required within specified timeframes.
