Position Overview:
We are seeking a Patient Care Manager to oversee operations at our hospice site.
You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager.

Your responsibilities will include managing daily operations, overseeing the clinical components of the hospice program, and ensuring high-quality care for patients.

You will also be responsible for managing the site in the absence of the Executive Director, Administrator, or Senior/Executive Patient Care Manager.
Key duties include supervising clinical and administrative functions, maintaining accurate records, responding to inquiries with professionalism and compassion, and participating in on-call nursing schedules while providing direct bedside care to patients in their homes.

Candidate Profile:
Education/Experience: A Registered Nurse with at least an Associate's Degree in Nursing from an accredited institution is required. A Bachelor's Degree in Nursing is preferred.
A minimum of three years of direct patient care experience in a clinical environment is essential. Additionally, at least one year of management or supervisory experience is required, along with two or more years of experience in hospice or home care settings.

Current nursing license in the state of employment is mandatory, along with CPR certification, valid automobile insurance, and a driver's license.

Personal Traits, Qualities, and Aptitudes: A deep sensitivity to the emotions of terminally ill patients and their families is crucial, both before and after the patient's passing.

Flexibility is key, as the role requires the ability to perform various tasks efficiently and maintain composure under changing circumstances.

Strong communication skills are necessary to engage effectively with individuals from diverse socioeconomic backgrounds. The ability to influence opinions and make informed decisions based on critical assessments is essential. Advocacy for patients is a vital component of this role.

Professional conduct, maturity, cooperation, adaptability, and tact in interactions with colleagues and all members of the hospice team are expected.


Required Knowledge and Skills:


A solid understanding of the needs of terminally ill patients and their families, along with knowledge of the hospice model, is essential. Familiarity with the roles of various disciplines providing hospice services, excellent patient assessment capabilities, and strong oral and written communication skills are required. A thorough understanding of managed care principles and regulatory guidelines (including Medicare and Medicaid) is also necessary.
