Key Responsibilities- Community Support: Interact effectively and respectfully with consumers in their home and community, promoting a positive and supportive environment.
- Relationship Building: Support and promote relationships with family and friends, while maintaining a family culture within the home and community.
- Collaboration: Work effectively with community partners, including Social Workers, Instructors/Coaches, Day Program Staff, and Court Personnel, to provide comprehensive support services.
- Community-Based Opportunities: Support and promote community-based opportunities according to consumer preferences, enhancing their quality of life.
- Team Collaboration: Contribute positively to team meetings and special/social events, ensuring effective communication and collaboration.
- Professional Development: Avail yourself of additional relevant training according to personal interests and/or manager recommendations, enhancing your skills and knowledge.
- Training and Orientation: Assist with the orientation and training of new staff, ensuring competent and confident employees who prioritize the health, safety, and comfort of our beneficiaries.
- Psychoeducation: Provide psychoeducation as indicated in the Person-Centered Plan, promoting informed decision-making and empowerment.
- Crisis Intervention: Assist with crisis interventions, de-escalating situations and promoting a safe environment.
- Support Services: Assist consumers in attending professional appointments, life skills training, independence, and rehabilitation, as necessary according to the Person-Centered Plan.
- Community Linkage: Ensure linkage to community services and resources, enhancing our beneficiaries' access to support and care.
- Behavioral and Substance Abuse Interventions: Assist the Team Leader with behavioral and substance abuse interventions, promoting healthy behaviors and outcomes.
- Person-Centered Planning: Participate in the initial development, implementation, and ongoing revision of the Person-Centered Plan, ensuring tailored support services.
- Progress Reporting: Communicate the recipient's progress and the effectiveness of strategies and interventions to the Team Leader, as outlined in the Person-Centered Plan.
- Recreational Activities: Organize and participate in recreational/social activities with beneficiaries, promoting socialization and enjoyment.
- Record Keeping: Maintain accurate records and charting on each individual, reporting unusual and critical incidents in a professional and timely manner.
- Medication Familiarization: Familiarize yourself with medications used by clients and policies regarding medication administration.
- Electronic Medical Records: Accurately document all billable encounters into Southeastern Integrated Care's EMR system within 24 hours, ensuring accurate and timely record-keeping.
This position does not oversee staff.
Qualifications- Education/Experience: Minimum one year experience supporting adult individuals with behavioral or mental health challenges and/or substance abuse issues; High school diploma, GED, or equivalent.
- Required Skills/Abilities: Ability to write clearly and legibly; Schedule flexibility to meet the needs of beneficiaries; Knowledge of Behavior Management techniques, Substance Abuse counseling/support, and Recreation Leadership; Ability to provide guidance and training of life skills; Ability to implement Individualized Treatment Plans and monitor progress towards goals; Ability to interact effectively, including oral and written communications; A pleasant, positive nurturing personality who identifies the importance of the essence of home and enhancement of life.
- Certificates, Licenses, Registrations: Copy of a valid, unrestricted NC Driver's license, proof of insurance.
