LOCATION: Remote - must reside in or near designated counties in North Carolina. This is a home-based position that requires community travel.
GENERAL OVERVIEW
The Transition Coordinator QP (TC) plays a pivotal role in facilitating the smooth transition of individuals from institutional settings to home and community-based services. This position is dedicated to preparing members for discharge and providing support during their adjustment period post-discharge. The TC will engage with members in their communities, ensuring that services are effectively coordinated.
IMPORTANT NOTE: This role necessitates access to confidential healthcare information as per legal requirements, including HIPAA regulations. The individual will be required to undergo training on these laws and adhere to them as part of their employment responsibilities.
KEY RESPONSIBILITIES
Transition Planning:
- Manage an active caseload of members in transition planning.
- Collaborate with management to establish annual targets for successful transitions.
- Ensure completion of the Pre-Quality of Life survey prior to lease signing.
- Educate service providers about their roles and the TC's responsibilities.
- Maintain adherence to established policies and boundaries.
Monitoring:
- Receive monthly updates for transitioned members and submit necessary documentation.
- Collaborate with community providers to ensure adequate service delivery.
Transition Process:
The Transition Coordinator QP will work closely with the Transition Coordinator LP to ensure that individuals wishing to move to more inclusive settings receive appropriate behavioral health services. This includes forming a transition team and developing a comprehensive transition plan.
To ensure a successful transition, the TC will:
- Conduct meetings with members and review clinical records to complete necessary assessments.
- Assist members in creating a detailed plan that links them to essential services.
- Engage with members and their families to develop a holistic transition strategy.
- Implement person-centered planning processes that prioritize member involvement.
- Identify and secure community resources essential for a successful transition.
- Utilize motivational interviewing techniques to develop thorough plans.
- Facilitate communication among institutions, provider agencies, and community supports.
Diversion Responsibilities:
The TC will also address transition needs identified through the Department of Justice diversion process, assisting members in navigating the transition planning process.
Each transition is unique and may require multiple meetings and ongoing communication to ensure an organized process. The TC will establish a meeting schedule that meets the specific needs of each transition.
Documentation:
The TC is responsible for maintaining clear and concise documentation of the transition process for each member, which will inform relevant organizations and authorities.
Collaboration:
Ongoing communication with all parties involved in the transition process is essential. The TC will work closely with various departments to ensure successful transition planning.
QUALIFICATIONS:
A Bachelor's degree in a Human Services field and two years of relevant experience, or a Bachelor's degree in another field with four years of relevant experience.
PHYSICAL DEMANDS:
- Close visual acuity for document preparation and analysis.
- Physical activities include walking, talking, and repetitive motions.
- Sedentary work with occasional lifting of up to 10 pounds.
RESIDENCY REQUIREMENT: Candidates must reside in North Carolina or within 40 miles of the state border.
SALARY: Commensurate with qualifications and experience.
