Job Summary

We are seeking a highly skilled Integrated Care Coordinator to join our team at Health Connect America. As a key member of our care coordination team, you will play a vital role in delivering comprehensive, person-centered care to individuals and families in need.

Your primary responsibilities will include planning, coordinating, and monitoring individualized treatment plans to align with behavioral health goals. You will work closely with our Nurse Practitioner to facilitate clinic appointments and ensure seamless transitions of care.

In addition to your clinical expertise, you will also support marketing initiatives for new referrals and engage in outreach to integrated care attributed members. Your excellent communication skills and ability to educate clients on our program and facilitate enrollment will be essential in this role.

Key Responsibilities:

  • Assess, coordinate, and document client care plans
  • Provide comprehensive care management, coordination, and health promotion
  • Develop individualized, person-centered care plans incorporating assessment results and Division guidelines
  • Coordinate follow-up services for recent hospitalizations or life transitions
  • Establish collaborative relationships with care team members and community resources
  • Attend Treatment Team and supervision meetings, integrated care team meetings, and serve as a liaison with other professionals and agencies

Requirements:

  • Bachelor's degree in any discipline required; human services-related discipline preferred
  • Minimum of one of the following qualifications to meet criteria as a Qualified Professional (QP): RN with four years of experience, master's degree in a human service field with one year of experience, or bachelor's degree in a human service field with two years of experience
  • Two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience for care managers serving members with LTSS needs
