Peer Support Specialist

Job Summary:
Vaya Health is seeking a highly skilled and compassionate Peer Support Specialist to join our team. As a Peer Support Specialist, you will play a critical role in supporting individuals with serious mental illness (SMI) who are being released from a Department of Adult Corrections (DAC) facility.
Key Responsibilities:
- Coordinate with key providers and stakeholders to ensure seamless care coordination and treatment planning.
- Provide person-centered support and guidance to individuals and their treatment teams.
- Identify gaps in services and intervene to ensure individuals receive appropriate care and discharge planning.
- Collaborate with community stakeholders to promote positive outcomes and reduce high-risk events.
Requirements:
- North Carolina Certified Peer Support Specialist (NCPSS) Certification
- High school diploma or GED required and at least two years of progressive experience providing similar services to the populations served (MH/SU/IDD)
- Proficiency in Microsoft Office products (such as Word, Excel, Outlook, etc.)
Preferred Work Experience:
- Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal
