Key Responsibilities:

  • Conduct comprehensive medication reviews and provide expert recommendations to healthcare professionals.
  • Support patients in managing long-term conditions through effective medication optimization.
  • Engage in our innovative care home initiatives.
  • Collaborate with fellow Clinical Pharmacists across multiple practices.

The successful candidate will have the chance to enhance their professional skills through accredited training programs, including the CPPE diploma and non-medical prescribing qualifications, if not already obtained.

Role Overview:

The scope of this position is designed to evolve as the post holder progresses through their professional development, including obtaining relevant qualifications and gaining practical experience.

Clinical Responsibilities:

  • Perform clinical assessments for patients with complex medical needs, ensuring optimal use of medications.
  • Manage a caseload of patients with minor ailments while adhering to established practice guidelines.
  • Provide direct support to patients regarding their medication queries through dedicated clinics and a telephone help line.
  • Analyze and present medication data to identify issues and support informed decision-making.

Education and Training:

Facilitate training sessions for the primary healthcare team on therapeutic practices and medication management.

Qualifications:

  • Mandatory registration with the General Pharmaceutical Council (GPhC).
  • Post-graduate diploma level or equivalent experience in a relevant field.
  • Independent prescriber status or intent to pursue this qualification.
